WHAT YOU'LL DO:
As a Resource Specialist, you will organize and implement social services to patients post Emergency Department visit and acute care admission.
Within this role, you will receive referrals for individuals from at-risk populations from interdisciplinary team member including physicians, Case Managers, staff nurses and other members of the care team, and will be assigned to a team to follow through acute care continuum.

Estimated Salary: $20 to $28 per hour based on qualifications.
